Liability Waiver and Agreement for in-person training during COVID-19 Pandemic 

To physically train at the Movement Center during the COVID-19 Pandemic (GYROTONIC® Boston MetroWest at 93 West Street in Medfield, MA.) means that you fully agree to the following: 

I understand that I have enrolled in a program of potentially strenuous physical activity. In addition I understand that I am entering the building and studio at my own risk during the COVID-19 Pandemic.

As part of the agreement to undertake training during the COVID-19 Pandemic period I will read and observe all of the studio’s best-practices and safety-protocol guidelines (found here: http://gyrotonicbmw.com/covid-19-safety-protocol)

I also agree to reply to the session questionnaire that will be sent out via email the day before my session. We require honest responses to the questionnaire before EACH session.

I hereby absolve Deborah Saparoff, including her successors, assigns, employees and contractors, the studio GYROTONIC® Boston MetroWest, and the 93 West Street Building Management from any responsibility for injuries, adverse health effects or sicknesses that I might sustain or contract while visiting the studio or practicing these movements or methods, or any actions learned with or by Deborah Saparoff or GYROTONIC® Boston MetroWest. 

I hereby release for myself, my heirs and assigns, Deborah Saparoff and GYROTONIC® Boston MetroWest, from any claims, demands and causes of actions arising from my participation in an exercise program or my visits to the studio during the Pandemic.

Agreement to this Liability Waiver will act as your continued agreement to all ensuing sessions during the COVID-19 Pandemic.
